Brief Overview

This session provides general practitioners with an overview of how Back In Motion Clayton supports patients through the GP Chronic Condition Management Plan (GPCCMP). The program highlights how AHPRA-registered physiotherapists deliver Medicare-subsidised physiotherapy services to help patients manage chronic conditions, improve mobility, reduce pain, and enhance overall function.

Participants will gain insight into the referral process, communication pathways, and the value of integrated care in achieving better health outcomes. The session also outlines how collaborative reporting and progress updates between GPs and physiotherapists ensure continuity of care and patient-centred management.

Program Details

By participating in this activity, GPs and practice staff will:

  1. Enhance knowledge of chronic condition management – understanding how physiotherapy can support patients under GP Chronic Condition Management Plans (GPCCMP).
  2. Develop practical skills – applying referral pathways, coordinating care with physiotherapists, and monitoring patient progress.
  3. Improve attitudes toward multidisciplinary care – recognising the value of collaborative approaches in optimising patient outcomes.
  4. Strengthen patient engagement strategies – supporting adherence to physiotherapy programs and encouraging proactive management of chronic conditions. 

Program Outcome

Our activity addresses the gap in accessible, evidence-based physiotherapy care for patients with chronic conditions. Many patients under GP Chronic Condition Management Plans (GPCCMP) face challenges in accessing tailored physiotherapy support to manage pain, improve mobility, and maintain function. This activity equips GPs and patients with the knowledge of available subsidised physiotherapy services and ensures patients receive individualised, outcomes-focused care, bridging the gap between primary care management and specialised physiotherapy intervention.

By completing this activity, participants will:

  1. Understand the GP Chronic Condition Management Plan (GPCCMP) – including eligibility criteria, referral process, and Medicare-subsidised physiotherapy services.
  2. Recognise the role of physiotherapy in chronic condition management – including pain management, mobility improvement, functional restoration, and prevention of further complications.
  3. Learn how to integrate physiotherapy into patient care plans – including effective communication with physiotherapists, monitoring progress, and coordinating ongoing care.
  4. Identify barriers and solutions for patient engagement – including strategies to improve adherence to physiotherapy programs and optimise health outcomes. 

Our activity is designed to ensure participants achieve the learning outcomes through a combination of interactive and practical methods, including: Case-based discussions – exploring real patient scenarios to demonstrate the integration of physiotherapy into chronic condition management. Guided presentations – providing clear explanations of GP Chronic Condition Management Plans (GPCCMP), eligibility, and referral pathways. Practical demonstrations – showing evidence-based physiotherapy interventions and strategies to improve mobility, manage pain, and enhance function. Collaborative problem-solving – participants work through common barriers to patient engagement and care coordination, reinforcing key concepts. Resource sharing – providing take-home materials, referral templates, and progress monitoring tools to support application in clinical practice. These methods combine knowledge acquisition, skill development, and practical application, ensuring participants can confidently integrate physiotherapy into patient care.

We evaluate our activity through participant feedback and reflections, including surveys on relevance and usefulness. Feedback is used to refine content, teaching methods, and resources to ensure participants gain practical knowledge and skills for integrating physiotherapy into chronic condition management.
